A referral may enable you to access government subsidised services.

Please complete any pre appointment information requested by your Dietitian.

Bring with you 

- A list of you questions and concerns.

- A list of your medications.

- Any recent blood test results (including your blood glucose records if you have diabetes).

- A completed food diary or food and symptom diary (if requested)

We encourage you to bring your partner/ guardian/ child or other support with you.

We want you to get the most from our services.

We just ask that you let us know who will be present for the appointment.

If you are living in the Upper Spencer Gulf, Eyre Peninsula, Eastern Eyre Peninsula, Roxby Downs, Flinders Ranges, Port Pirie, Port Lincoln or anywhere in rural and remote South Australia and you are interested accessing our services, give us a call. 

Clients can access financial support for Dietitian appointments through a range of government and private avenues. Some examples are listed below.

If you are ineligible for government funded services and are experiencing financial hardship, contact us.

GP's Chronic Condition Management Plan 

Clients with this referral are eligible for a $61.80 rebate per appointment through Medicare.

GP Chronic condition management plans must be directed to Gulf Dietetics and state how many sessions (out of 5) are allocate to Dietetics for a calendar year.

GP referrals for Complex Neurodevelopmental conditions and eligible disability

Clients aged under 25 years of age with this GP referral are eligible for a $87.25 rebate per appointment through Medicare.

Private Health insurance- Extra's cover check with your insurer if they cover Dietetics.

Department of Veterans' Affairs.  If you are a DVA Gold or White Card holder you may be eligible for services if you have an assessed clinical need and are referred via your GP under the Allied Health Services treatment cycle.
